Insurance

It is your responsibility to know what your insurance covers and if Park View Pediatrics is in network with your company. 
A current insurance card for all existing policies must be presented at every appointment. If we don't have the appropriate insurance on file, the payment for that visit may fall to you. 

Copays are due at the time of service along with any balances that have been assigned to the patient after insurance has paid their portion. 
If you are uninsured, please ask to speak with our office manager about payment options.
